Meet & Greet Forms

After deciding which service best suits you and your precious pets, you're ready for our meet and greet. This initial consultation gives me the opportunity to meet you and your pets and discuss their specific needs and dislikes. This visit is FREE and is required prior to service for any New Client. I know that you have a busy schedule and, therefore, I am committed to keeping the initial meet and greet less than 30 minutes. To make the process as smooth as possible, please review the following information before our arrival:

Pet Care Registration

Prior to service, please sign up online using our new online registration.

Or you can download, print, and sign the following forms: Client Information, Pet Profile, and Service Agreement
 If you'd prefer to fill out the forms during the meet and greet, we're more than happy to go over all the information at that time.

Copy of House Keys

Please have a copy of your house key available so that way we may enter your home to provide pet care. In case of an emergency, a spare key on property(safely hidden) allows Pollywog's Pet Sitting access to your pet for care. Please test the copied keys to make sure they work properly with the corresponding locks.

Payment Arrangements

Payment must be made in full prior to service. Currently, we accept either cash, personal checks addressed to Pollywog's Pet Sitting, or credit card, payed through invoice by STRIPE.
